Ohio law requires Ohio school districts to report on each building, each grade kindergarten through 12, on the types of prevention-focused programs, services and supports used to assist students in developing the knowledge and skills to engage in healthy behaviors and decision-making and to increase their awareness of the dangers and consequences of risky behaviors, including substance abuse, suicide, bullying, and other harmful behaviors.
The following survey provides a template that guides districts compliance with reporting and enables the Ohio Department of Education to collect data on prevention-focused programs, services and supports implemented in Ohio schools.
