The form below should be used to submit only students who were missed entirely for the Summer 2021 P-EBT program. Please note: Summer P-EBT has a much broader scope and you should review the eligibility criteria below before submitting. This form will be open through 4:00 pm on September 9 and the students will be funded after that date for the Summer Benefit. This will be the ONLY opportunity to submit students who were missed for Summer. If you have more than 10 students to submit, please contact support at pebt-support@ohio-k12.help.
