Restraint & Seclusion Incidents Reporting
for School Year 2024-2025

Ohio Administrative Code 3301-35-15 (K) requires districts to annually submit a reporting of incidents of student restraint and seclusion to the Ohio Department of Education and Workforce. Participation in this data collection fulfills your district’s reporting requirement of the above stated rule.

Districts that do not participate in this data collection must meet their reporting requirement by alternative means.
